DMA (“Deese Marketing Association”) is committed to resolving disputes with its customers in a fair and efficient manner, through customer service, and if necessary, by arbitration. If you do not wish to participate in arbitration pursuant to Deese Marketing Association’s Payment Agreement for Zero % Interest (“Financing Schedule) and wish to opt out of it, please completely fill out this form, sign and date it, and mail it to us within 60 days from the date you first use the Deese Marketing Association’s services or otherwise accept the Zero % Interest (“Financing Schedule) Terms.
